About the Role
This is an exciting opportunity to join a newly formed, specialist Continuing Healthcare (CHC) Team as a CHC Senior Practitioner, where you will play a key role in ensuring delivery of high-quality assessments and support for individuals with complex health and care needs.
In this role, you will:
• Lead and contribute to CHC assessments, reviews, and Decision Support Tool (DST) processes
• Work collaboratively with colleagues, NHS partners, Integrated Care Boards (ICBs), providers, and families to deliver person-centred outcomes
• Provide professional advice and guidance on CHC processes and the National Framework
• Ensure accurate, timely recording and robust decision-making in line with best practice and statutory requirements
• Support effective case management, including identifying and managing risk
• Contribute to service development, quality assurance, and continuous improvement activity
• Represent the local authority at multi-agency meetings, panels, and where required, within dispute resolution processes
• Identify safeguarding concerns and escalate appropriately, ensuring the wellbeing of vulnerable adults
About the Team
Haringey’s Continuing Health Care Team is a newly established, specialist service within Adult Social Care, created to strengthen and embed high quality CHC practice across the borough. The team is made up of skilled practitioners who work at the interface of health and social care, supporting residents with complex, long term health needs. We work in close collaboration with NHS partners, providers and internal services to deliver consistent, transparent and person centred CHC processes.
About You
We are looking for a qualified and SWE-registered Social Worker with strong post-qualification experience and a proven commitment to strengths-based practice. You will bring a solid understanding of the legislative and policy frameworks underpinning Adult Social Care, alongside experience of working within multi-agency environments to support individuals with complex needs.
We are looking for a motivated and knowledgeable practitioner who:
• Has experience supporting adults with complex, long-term, or unpredictable health and care needs
• Demonstrates a strong understanding of the CHC National Framework and multi-agency working
• Is confident managing and contributing to complex assessments and decision-making processes
• Experienced in supporting others in improving social care practice.
• Builds positive, professional relationships with a range of stakeholders
• Has strong analytical skills and the ability to assess and manage risk effectively
• Communicates clearly and confidently, with good negotiation and influencing skills
• Is well organised, able to manage competing priorities, and work both independently and as part of a team
• Is committed to delivering high-quality, equitable services and continuous improvement
